Case study #2 male w/Sciatica:,[link widoczny dla zalogowanych]
Dennis a 41-year old male presented with Sciatica. He had had sciatica on and off for the last three years after he had injured his back playing soccer. Dennis worked as a truck driver and the sciatica was a real problem because when he had a flare up it was nearly impossible to drive for any distance. The pain would shoot down his left leg and he described it like a lightening bolt. I examined the MRI and it showed he had a L5-S1 3mm disc bulge,[link widoczny dla zalogowanych], as well as a L2-L3 2mm disc bulge. He had a trigger point muscular spasm in his left buttock as well as one just above the top of his sacrum. I injected his trigger points as well as the ligaments surrounding the weakened joints. After 3 weeks Dennis reported feeling about 70% better. He was able to drive without pain and only experienced stiffness after sitting for a long time. He returned 3 months later for another injection and has reported feeling 90% better overall.

Prolozone is a form of non-surgical ligament/tendon reconstruction and is a permanent treatment for chronic pain. The therapy works by injecting collagen producing substances into the areas of damaged connective tissue and allowing the tissue to rebuild. This injection therapy causes the formation of new ligament tissue in areas where it has become weak. Pain will persist as long as the connective tissue remains damaged. The response of treatment varies from individual to individual,[link widoczny dla zalogowanych], and depends on one’s healing ability. Shari a 35-year old female presented with a chief complaint of fatigue,[link widoczny dla zalogowanych], and chronic back pain. Her back had been in spasm since a car accident 5 years earlier. She had two small children and was upset because she couldn’t do things with her children that other moms could because she was always in pain. She had gotten in the habit of having 3 martinis every afternoon to take the pain away. She had a home sewing business but could only sit at her sewing machine for 1 hour at a time before the pain started. I looked at her lumber MRI and it showed a 5 mm disk herneation at the L5-S1 and a 3mm disc bulge a the L4-L5 vertebrae. When I examined Shari’s back she had localized pain at the areas of the disc bulges as well as multiple trigger point areas where her back muscles were spasming in order to protect the area of injury. I injected her trigger points to reduce muscular spasm and then injected bilaterally the ligaments surrounding each disc bulge. Shari felt fine following the treatment,[link widoczny dla zalogowanych], I told her to go home and rest for 2 days without taking any anti-inflammatory pain medications. When I saw her 2 weeks later she said that she had been to Disneyland with her kids and had been waiting for the pain to start but it never did and she just kept walking and walking. She had stopped needing the martinis in the afternoon and felt 90% better. I have seen Shari over the last 12 months as we have been treating her fatigue, her back pain has never returned. She now can sit and sew for 5 hours at a time.

Many instances of chronic joint pain are related to connective tissue damage or injury. The most common connective tissues involved are ligaments or tendons; this is usually called a sprain or a strain. When a strain/sprain occurs the ligaments and/or tendons are separated from the bone. Ligaments are the structural “rubber bands” that hold bones to bones in joints – acting like the body’s shock absorbers. Ligaments can become weak or injured and may not heal back to their original strength or endurance. Ligaments will also not tighten on their own to their original length once injured. This is largely because the blood supply to ligaments is limited, and therefore healing is slow and not always complete. To further complicate this, ligaments also have many nerve endings, and therefore the person will feel pain at the areas where the ligaments are damaged or loose. Often the site of injury takes much longer to heal and the patient complains of never feeling 100%. This is because connective tissue has a limited blood supply. Healing components of the blood slowly move into these areas and it becomes difficult for these tissues to receive what they need to properly repair. If the connective tissue remains weakened and overstretched then repeat injury occurs. A perfect example of this is a sprained ankle. If the ankle does not completely heal, it remains weak and continues to roll or re- sprain itself. In the British Journal of Sports Medicine,[link widoczny dla zalogowanych], it is noted that most connective tissue injuries are degenerative and not inflammatory in nature. (Kader de et al 2002).
This completely changes the conventional anti-inflammatory treatment approach to chronic pain, where chronic pain is most commonly treated with steroid injections and anti-inflammatory pain medications. A pain management approach that does not address connective tissue damage is limited in the scope of how well it can actually treat the underlying cause of the pain and repair the connective tissue within the joint.

Another Transformational Leadership trait is building leaders on teams. Building leadership skills is a key to delegation. If there are good communication standards,[link widoczny dla zalogowanych], then there can be minimal conflict in the workplace due to misunderstanding or missing information.
A clear vision is followed by clear goals and a solid communication plan. Communication consists of two functions: 1) Delivery and 2) Reception. We must be aware of our words when speaking. There is power in the choice of words. The wrong choice of words will communicate a different message than intended,[link widoczny dla zalogowanych], so choose the words for communicating very carefully. Along with the choice of words come the other factors in communication – the inflection in the voice,[link widoczny dla zalogowanych], the physical stance and posture, and certainly the pace of the conversation. As an effective Transformational Leader be aware that there are important elements in every communication. Be active in listening. Be careful in your choice of words. You don’t have to respond immediately once the other person stops talking. In fact, leaving a moment of silence lets the person talking know that you listened and are thinking about what was said. The moment of silence can be a validation that you have listened.

4. YOU ARE IN CONTROL
You can choose the language you use and it can be simple. Always remember you are in charge of what comes out of your mouth, pen and typing. However, you may receive emails that are complex, so it is good to know and understand complex English as well. You may also receive emails that are difficult to understand. This may not be a lack of understanding on your part. It may be because the writer is not using the best English. Here are a number of problems contained in emails:

Gallup and now Marcus Buckingham on his own, have both done a great deal of research in this area and they have proven time and time again that encouraging people to play to their strengths and praising them for doing a good job is far more effective than focusing on what the employee needs to improve upon.
I’m a Green Bay Packers fan, so I’m very very happy this week (for those of you who don’t follow American Football, they just won the Super Bowl). Win or lose, I’m a proud Packers fan, or a Cheesehead as we are sometimes called. I grew up in Wisconsin, my Dad grew up 30 minutes out of Green Bay. I was called “Packer” before I was born and they knew what to name me. I ride a motorbike. Well to be fair, it’s a little 150 cc scooter, but I think I’m pretty bad A$$! I tell you this because, getting a motorbike license in Sydney is a very rigorous process and my experience demonstrates well the difference between focusing on what you do wrong versus praise.

Fast forward a few months to my retake. Guess what? It was raining again!!! But what happened served to remind me of the power of positive reinforcement. This instructor was just the opposite of the first one I had. He made me feel like I was the best rider he’d ever seen. Consciously I KNEW this not to be true, but unconsciously I was almost giddy by the time I took the practical. There was no way I could fail. I knew it.

Conclusion
The processing of an absolute first involves the hydrocarbon solvent extraction of a 'concrete' from the plant material, a semi-solid mixture of typically 50% wax and 50% volatile oil. The concrete is again processed using ethyl alcohol (the same alcohol found in beer, wine, etc.) in which the wax is only slightly soluble. The volatile plant oil separates into the alcohol and this mixture is removed. The alcohol is then evaporated and the result is an almost pure plant extract – depending on the care taken in the evaporation process, sometimes 2% or less of the ethyl alcohol may remain. The use of solvents in the extraction process notwithstanding, absolutes can have incredibly deep and complex aromas.
A number of factors determine the final quality of a steam distilled essential oil. Aside from the plant material itself, most important are time, temperature and pressure, and the quality of the distillation equipment. Essential oils are very complex products; each is made up of many, sometimes hundreds, of distinct molecules which come together to form the oil's aroma and therapeutic properties. Some of these molecules are fairly delicate structures which can be altered or destroyed by adverse environmental conditions. So, much like a fine meal is more flavorful when made with patience, most oils benefit from a long, slow 'cooking' process.
Some plants, and particularly flowers, do not lend themselves to steam distilling. They are too delicate, or their fragrance and therapeutic essences cannot be completely released by water alone. These oils will be produced as 'absolutes' – and while not technically considered essential oils they can still be of therapeutic value. Jasmine and Rose in particular are delicate flowers who's oils are often found in 'absolute' form.
CO2's, with some obvious advantages,[link widoczny dla zalogowanych], are not always the best choice for a particular need. They still are the most expensive,[link widoczny dla zalogowanych], despite their higher yields. The resultant product differs slightly compared to one produced another way – the oils produced by steam distillation of some plants may sometimes be found to have a more agreeable aroma. Patchouli, for example,[link widoczny dla zalogowanych], seems to benefit from the steam distillation process by becoming a little warmer and richer. Many other essential oils are quite effectively produced via steam distillation, with little alteration from the original plant state. Oils from other plant species do seem more 'complete' with CO2 processing, with Frankincense and most of the 'spice' oils being good examples where a little something special is present in the aroma.
Cold Pressing
Steam distillation, the most common method of essential oil production, involves the flow of steam into a chamber holding the raw plant material. The steam causes small sacs containing essential oil to burst. The oil is then carried by the steam out of the chamber and into a chilled condenser, where the steam once again becomes water. (Hydro-distillation is a similar process where the plant material is boiled, with the resultant steam being captured and condensed). The oil and water are then separated; the water, referred to as a 'hydrosol', can be retained as it will have some of the plant essence. Rose hydrosol, for example,[link widoczny dla zalogowanych], is commonly used for it's mild antiseptic and soothing properties, as well as it's pleasing floral aroma.

Absolutes
Producing essential oils of aromatherapeutic grade is skill requiring years of experience. It takes the work of a dedicated artesian at every step,[link widoczny dla zalogowanych], from growing and harvesting to fine-tuning the distillation process, to produce a truly fine oil. The making of a fine essential oil relies far more on knowledge and experience than it does on the particular extraction method. There are, however, legitimate reasons to select one distillation method over another – some plants simply require a particular process to produce a fine oil,[link widoczny dla zalogowanych], and the oil needed for a particular application may only be made by one process. In the end, as is often the case in aromatherapy, your own sense of smell can tell you which oil will work best for you.
Steam and Hydro Distillation
And now for the most modern technologies, Carbon Dioxide and Supercritical Carbon Dioxide extraction. Both methods involve the use of carbon dioxide as the 'solvent' which carries the essential oil away from the raw plant material. The lower pressure CO2 extraction involves chilling carbon dioxide to between 35 and 55 degrees F, and pumping it through the plant material at about 1000 psi. The carbon dioxide in this condition is condensed to a liquid. Supercritical CO2 extraction (SCO2) involves carbon dioxide heated to 87 degrees F and pumped through the plant material at around 8,[link widoczny dla zalogowanych],000 psi – under these conditions, the carbon dioxide is likened to a 'dense fog' or vapor. With release of the pressure in either process, the carbon dioxide escapes in its gaseous form, leaving the essential oil behind.
The temperature of the extraction chamber cannot be too high, lest some components of the oil be altered or destroyed. The same is true of the chamber's pressure. Lavender, for example, should not be processed at over 245 degrees F and three pounds per square inch of pressure (3 psi). Higher temperatures and/or pressures result in a 'harsh' aroma – more chemical than floral – and lessen the oil's therapeutic effects. Also, the extraction period must be allowed to continue for a certain period of time in order to flush ALL the oil's components from the plant, as some are released more quickly than others.
New methods of essential oil extraction are entering the mainstream of aromatherapy, offering new choices in oils never before available. With the new labels of 'CO2' and 'SCO2', along with the traditional 'steam' and 'hydro' distillations, 'absolutes', and 'cold pressing', a little education for the aromatherapy enthusiast can go a long way in essential oil selection. Is one process better than another? Does one produce a nicer smelling oil, or one with greater aromatherapeutic value? It turns out that essential oil production, like winemaking, is an art form as well as a science. The value of the newer processing methods depends greatly on the experience of the distiller, as well as the intended application of the final product. Each method is important, and has it's place in the making of aromatherapy-grade essential oils.
Finally, there is the 'cold pressing' of citrus oils from the peels of fruit, as is done with Bergamot, Orange, Lemon, and the like. This method involves the simple pressing of the rind at about 120 degrees F to extract the oil. Little, if any,[link widoczny dla zalogowanych], alteration from the oil's original state occurs – these citrus oils retain their bright, fresh, uplifting aromas like that of smelling a wonderfully ripe fruit.
Despite the drawbacks of aggressive processing, high temperatures and pressures are often used to produces large quantities of oil in a short period of time. These oils are usually destined for use in cosmetic and processed food manufacturing, but are sometimes sold to final consumers as essential oils for use in aromatherapy. These oils will be less expensive, but are of limited therapeutic value, and the difference is apparent when the aromas are compared side-by-side.

These carbon dioxide methods have a couple of advantages: Like steam distillation, there are no solvent residues left behind, and the resultant product is quite pure. Like solvent extraction, there is no heat applied to the plant material or essential oil to alter it in any way. The oil produced is very accurate with respect to the original state of the plant. The CO2 methods also are the most efficient, producing the most oil per amount of plant (one of the reasons for the high cost of essential oils is the low yield of oil from most plants – one ton of Rose petals produces less than 1 pound of oil, for example). The efficiency of CO2 extraction is particularly important when rare or endangered plant species are involved,[link widoczny dla zalogowanych], such as Indian Sandalwood – less of the precious plant is needed to produce an equivalent amount of oil.
